To Be Quilted Form

This form should be included with your quilt when placing in the To Be Quilted bin.  We highly encourage you to quilt your own quilts, but know that sometimes this is not possible.  We kindly request that you take back your quilts for binding, labeling, and washing.  Consider laminating a copy of this form and using a wet erase marker to reuse for several quilts.
